I don't have any information on what revision did it but I can confirm the copy I had as a kid crashed if you died to the Rainbow Clown/Mime boss fight. Specifically it brought up the error screen for trying to play a Game Boy Color game on an original Game Boy. I honestly thought it was a frustrating joke since the mime "drained the color" from you, but it was obnoxious since it was an ultra hard Game Over.
