Chainmail Armor
Chainmail armor was craftable in early Indev, but has since been eliminated. It's basically the same as leather or gold armor. Oddly, if you hack fire blocks into your inventory, you can make it yourself in current builds.
Sponge
The sponge block from Classic is still present in the game, although it has no use and cannot be crafted or obtained through regular means. It can be hacked into one's inventory, given to a player under SMP using /give or obtained semi-legitimately in the Creative mode (introduced in Beta 1.8), however it no longer works, instead behaving like a standard block.
Locked Chest
The Beta 1.4 update introduced "Steve Co. Supply Crates" that would be created in newly-generated chunks. It was a silly little April Fools' Day prank and a silly little Team Fortress 2 reference! The chests were removed in the 1.4_01 patch, but can still be placed using inventory hacks.
The locked chest block can be destroyed in one hit, and disappears over time. Additionally, it generates light equivalent to a Glowstone block.
Not surprisingly, when the storage chests were updated to use smaller, animated models in Beta 1.8, the locked chests were not changed.
Giant

A huge Zombie that has a ton of health and deals a ton of damage. Giants were added in the last version of Survival Test but haven't been used since. They can still be spawned through hacks, however.
The Other Music Discs
The 1.0.0 release added music disc items for all of the unused songs (except for where are we now, due to problems with spacing in the filename). Unfortunately, the items can't actually be obtained through normal gameplay, and can only be found in Creative mode. What a shame.
Ferns
Another case of an item that can only be obtained in Creative mode, the fern plant actually did appear naturally in Beta 1.6 and 1.7, appearing in swamp and rainforest biomes. Because the swamp biome was dramatically changed and the rainforest biome completely removed, these things don't generate anymore.

Unused graphics
Purple Arrow
Bundled with the standard arrow graphics are these odd purple arrows. These are actually a leftover from Survival Test, where the Skeleton's arrows were purple. In all subsequent versions, Skeleton arrows use the same graphics as the player's arrows.
Quiver
This was actually used in early Indev, but is no longer in use. As long as you can carry arrows without a quiver, it wouldn't be very useful.
Gray Enderdragon
The preliminary textures for the Enderdragon boss, seen in older screenshots. The used file is ender.png. The only difference between dragon.png and body.png is that the latter lacks the scales.
Steve the Testificate
The default player skin remapped to fit the Villager model, presumably as a template. Actually loading it in-game produces a less than desirable appearance. Note that this texture is separate from the actual villager textures, which are in their own subfolder.
Pigman
A variation of the Zombie Pigman that...isn't a zombie. It's been in the game for a while now, but still isn't used.
Basic Villager
A green-robed villager skin. Although this is probably supposed to be used, it isn't. The standard villagers use the farmer.png skin instead.
Footprint
A weird little square thing. The file name says it all.
Unused Map Icons
Some extra icons for the map item. The white arrow in the top left corner is the only one used.
Snow Golem's true face
The snow golem mob has this goofy face as the front texture for its head. Since the snow golem's head is always obscured by a pumpkin, the face is never seen.
