Regional Differences

Given that the Japanese version was released several months after the US version, it recieved a fair amount of touch-ups.
Photo Album
The photo album icon on the main menu gets a flashing NEW! icon whenever there are new photos to view.
Original 1985
While the English version starts you as small Mario whenever you restore a game, the Japanese version saves your current powerup. This opens up a bug where if you save at the right time while being hit and reload, you can end up as a small fire Mario.
The Game Over options in the English version ask you if you want to continue, and if not, if you want to save. In the Japanese version you get a single menu with Continue, Save, and End. In addition, the Save option has a big RANK IN! starburst if you have a score that can reach the record table.
Speaking of the high score table, pressing Start in the Japanese version lets you reset the high score board. Also, in the English version, when you reached the point totals to dethrone Luigi or Boo and unlock the repective modes, you needed to get your high score on the board by dying before the modes will unlock. In the Japanese version, they're unlocked as soon as you hit 100,000 or 300,000.
Challenge Mode
The point totals for getting a score medal have been reduced in the Japanese version for each of the first four worlds, except for 2-3 and 3-4. The score bar at the bottom of the screen fills to the new total.
You Vs. Boo
In the English version, while pressing Select allows you to switch between forms, you are stuck playing as either Mario or Luigi based on whatever you used last in other modes. In the Japanese version, you can cycle through the brothers at the same time as the forms. In addition, the Japanese version keeps track of your record time for each stage.
