The Binding of Isaac: Repentance is the third and final (for real this time) expansion pack for The Binding of Isaac: Rebirth. It features tons of brand-new content, much of which is integrated from The Binding of Isaac: Antibirth, a fan-made expansion that was originally released as a mod for Rebirth.

Unused Graphics
Edith
Edith was a planned "secret character" for Antibirth. She was incomplete at the time of the mod's release, and thus went unused, though she could be accessed through hacks. She does not appear as a character in Repentance, but one of her gameplay concepts was instead given to the Tainted Forgotten, and she makes a cameo appearance as a random statue the player can transform into when using an inverted The Chariot card.
Edith's Cursor
In Antibirth, Edith's main gameplay mechanic was that she could only move by teleportation, the position of which is controlled by the player and marked with a cursor in a similar fashion to Epic Fetus. This cursor graphic is new to Repentance, as a generic red cursor was used in Antibirth instead.
Tainted Edith Door
The graphics for the tainted character selection menu contain an unused character door, which appears to have been intended for Edith due to the salt shaker. Perhaps Edith was intended to return in Repentance with a tainted version at some point?
Dark Esau
A stage transition/Vs. screen portrait for Dark Esau. This may have been removed because Dark Esau is considered to be an enemy (or, more accurately, a miniboss) rather than a playable character.
Placeholder Item
A placeholder item icon. Why this didn't exist until Repentance is a mystery.
Pill Crusher
The Pill Crusher was an item that was originally planned for Antibirth. According to a blog post, it would have been a no-charge active item found in shops that, when used, would consume Isaac's held pill (if any) and activate a room-wide effect depending on the type of pill.
The file name of the item's icon indicates that it would have occupied the item ID 648, placing it between 4.5 Volt (647) and Fruity Plum (649). In the final game, this item ID goes unused.
Menorah
The entire sprite atlas of the Menorah familiar from Antibirth. The Menorah is one of the few (non-redundant) items from Antibirth that didn't make the cut for Repentance.
Stone Bombs
Graphics of live Stone Bombs (both normal and enlarged from Mr. Mega) from Antibirth. Stone Bombs are another of the few (non-redundant) items from Antibirth that didn't make the cut for Repentance.
Knife Pieces
The Knife Piece 2 and Knife Piece 3 familiars from Antibirth. The second and third knife pieces were combined in Repentance, leaving these graphics unused.
Beth's Heart
Beth's Heart is a familiar-based item that didn't make it into either Antibirth or Repentance.
"Pacifist" Costume
An unused sprite atlas, which, according to the file name, is a costume for a nonexistent item called "Pacifist" with ID 662. The animation file associated with this costume positions the flower on Isaac's hand, giving the appearance of Isaac holding onto it.
Fetus Tears
A sprite atlas for... "fetus tears"? These may have been intended for the unfinished item C Section, which has the description "Fetus shots".
Maid in the Mist
An unused boss portrait, apparently for a boss named "Maid in the Mist" according to the file name. This was likely an early concept for what would become Min-Min, as not only do they bear many visual similarities, but the file name for this portrait uses the enemy ID 913, which is instead used for Min-Min in the final game.
Teleporter Room Icon
A map icon for the teleporter room, which was used in Antibirth to access the back half of the Mausoleum. Both the teleporter room and the split floor system of the Mausoleum were cut from Repentance.
Planetarium Chance Icon
A "Found HUD" icon for Planetariums. Presumably, this icon would have been used to show the chance of a Planetarium generating on the current floor, a stat that is otherwise not visible to the player.
Red TNT
A sprite atlas of Red TNT, which was used in Antibirth and functioned as a more volatile variant of TNT.
Rotten Teratoma
There is an unused spritesheet for the rotten teratoma balls, that, judging by it's name, the looks and it's location near the used rotten fistula spritesheet suggests that The Mother boss was supposed to summon the teratoma as well as the fistula to fight Isaac, which doesn't happen in the release.

Unused Bosses
Raglich
- Enemy ID: 919
- Boss ID: 98
- Base HP: 792

- Champion: No
Raglich is a boss that appears to be a member of the Rag Man lineage. It does not appear in any rooms, meaning it can only be spawned with the debug console. Its portrait does not have a shadow, which implies it was cut before the revised Vs. screen was implemented. Raglich has three phases and a rather barebones attack pattern:
- Upon spawning, summons two Rag Creeps.
- Floats along the walls of the room, eventually stopping to fire a purple Brimstone beam. Despite being colored purple, this beam does not home in on Isaac, and it can kill the Rag Creeps it spawns.
- This is followed up with a quick dash to the opposite wall.
- Occasionally spawns a Rag Creep.
Raglich enters its second phase at approximately 66% health. Upon performing its dash attack, it will stop to shoot a volley of four purple fires in the diagonal directions, followed by another volley of fires in the cardinal directions.
At 33% health, Raglich will summon a pair of giant arms from below that rip out three columns of tiles on both sides of the arena, leaving only a small area to move. If Isaac lacks flight and is standing on a tile as is being removed, he will fall into the pit, take damage, and respawn after a few seconds, much like stepping on a Pitfall. Raglich otherwise behaves normally during this phase.
Clutch

Clutch is a poltergeist-like boss that appears to have been intended for the Ashpit, due to its use of Clickety Clacks. Unlike Raglich, Clutch actually has rooms, which can be accessed using the debug console. He can be added to a floor's boss pool with mods. This boss is referred to with multiple different names—its boss rooms are named "The Possessor", but it is referred to as "Clutch" in the entities2.xml and bossportraits.xml configuration files and in various file names. Its portrait is misaligned does not have a shadow, which implies it was cut before the revised Vs. screen was implemented. It also does not have a boss title, which, in addition to the aforementioned naming oddity, suggests that its name was not finalized before being cut.
Upon entering the boss room, Clutch will immediately possess one of the four Clickety Clacks in the room, causing its face and color to change and shoot 8 bone shots in the cardinal and diagonal directions. The possessed Clickety Clack will chase Isaac aggressively, shooting volleys of three bones when Isaac is lined up with it. After enough damage is dealt, Clutch is ejected from the Clickety Clack and is stunned for a few seconds, after which Clutch revives any fallen Clickety Clacks to repeat the cycle. The Clickety Clacks in this boss fight do not immediately revive, and must instead wait for Clutch to revive them.
Upon losing approximately two-thirds of its health, Clutch will scream and turn the four Clickety Clacks into two bone orbitals each that orbit Clutch, while leaving indestructible purple fires where the Clickety Clacks once stood. Clutch will then dash directly to Isaac's location and rapidly shoot 6 volleys of 8 red shots, repeating this attack endlessly until the boss is defeated. The bone orbitals can be damaged and eventually destroyed by Isaac.

Corpse Eater/Carrion Rider
- Enemy ID: 239.100/239.101

Corpse Eater and Carrion Rider were enemies in Antibirth, which could appear in the Mausoleum and the Corpse. They wandered around the room, charging randomly and eating other monsters. Their behavior in Repentance is clearly unfinished, as they behave identically to Grub in every way (even spawning the same enemies on death) and their animations do not function correctly in-game.
Rag Creep

Rag Creep is an enemy summoned exclusively by the unused boss Raglich. It behaves very similar to Wall Creep, but it fires four homing shots at a time instead of three non-homing shots.
Split Rage Creep

Split Rage Creep is new to Repentance. It behaves very similar to Rage Creep, though it shoots two Brimstone beams at 30° angles instead of one straight beam, leaving a blind spot in the middle.
Blind Bat

- Champion: Yes
Blind Bat was another enemy in Antibirth, which could appear in the Caves, Mines, Depths, Necropolis, and the Cathedral. It hung from the ceiling, waiting until Isaac passed under it to strike. In Repentance, it has no behavior other than dealing contact damage, instead sliding around the room when hit like the unused Skinless Hush boss. As a result, it never changes from the first frame of its animation of hanging from the ceiling.
Exorcist
- Enemy ID: 832
- Base HP: 25
- Stage HP: 0
- Champion: Yes
While Exorcists do appear in Repentance during The Heretic's summoning ritual, they do not appear as standalone enemies like they did in Antibirth, where they could appear elsewhere in the Mausoleum. Despite this, Exorcists seem to be fully functional and even have updated sprites, though their behavior is very different from Antibirth:
- Wanders around the room, summoning a Lil' Haunt a few seconds after spawning. If the Lil' Haunt is killed, it will spawn another one after a few seconds.
- If any enemy is killed while an Exorcist is alive in the room (including summoned Lil' Haunts and other Exorcists), it will leave behind a red ghost, identical to those that appear when Isaac holds Vade Retro.
- Oddly, ghost-like enemies (Lil' Haunt, Wizoob, etc) do not normally leave behind red ghosts when Isaac has Vade Retro. They will only ever leave a red ghost behind if an Exorcist is present in the room.
- If there are any red ghosts, an Exorcist can walk up to them and revive them as Lil' Haunts.
- Revived Lil' Haunts will turn back into red ghosts when killed.
- Vade Retro can be used to prevent this by destroying the red ghosts.
- When all Exorcists in the room have been killed, any red ghosts remaining in the room will disappear unless Isaac has Vade Retro.
Interestingly, while Exorcists are not normally used, they do appear in the game's bestiary and in unused Greed Mode enemy waves. This implies that the enemy was cut late in development.
Strifer
- Enemy ID: 839
- Base HP: 25
- Stage HP: 0
- Champion: Yes
Strifer was yet another enemy in Antibirth, which could appear in the Mausoleum, strafing back and forth while shooting lines of four bullets. This enemy also has no behavior in Repentance. If spawned, only its lower body will appear properly.
Nightwatch
- Enemy ID: 842
- Base HP: 25
- Stage HP: 5
- Champion: No
Nightwatch was an invincible enemy in Antibirth, which could appear in the Mausoleum, patrolling the area. If they spotted Isaac, they would summon reinforcements to attack. This enemy has no behavior in Repentance, and can be damaged like normal.
Canary/Foreigner
- Enemy ID: 843/843.1
- Base HP: 16/18
- Stage HP: 2
- Champion: Yes
Canary and Foreigner are new to Repentance. Once again, these enemies have no behaviour.
There are multiple icons in the bestiary sprite atlas that seem to be related to Foreigner, suggesting that it would have been able to split into multiple parts.
Vessel
- Enemy ID: 858
- Base HP: 60
- Stage HP: 0
- Champion: Yes
Vessel was, once again, an enemy in Antibirth, which could appear in the Corpse. It wandered around the room, occasionally spawning Small Maggots and creating green creep. In Repentance, it has no behavior.
Curse of the Giant
Curse of the Giant was a curse introduced in Antibirth, which would force all regular rooms on the floor to be large rooms by combining multiple smaller rooms together. This curse exists in Repentance and is fully functional, but can only be accessed with the curse command. It may have been cut due to its unstable nature—some room combinations can result in strange layouts, broken puzzles, unfair enemy combinations, and other issues.
Mortis
The game files contain a few references to a scrapped alternate floor for the Corpse, called Mortis. There are no graphics associated with Mortis, and there is no stage defined for it in stages.xml.
- The
roomsfolder contains a .stb file for Mortis, but it only contains a single room with some Poison Minds. Greed Mode also has a layout for Mortis, containing only empty room layouts for the enemy arena. music.xmlcontains a placeholder entry for Mortis, simply named "not done". It uses the Womb's music as a placeholder.- Mortis corresponds to stages 7d and 8d. Attempting to go to these using the console causes the game to freeze. Doing this in Greed Mode instantly crashes the game.
Unused Greed Mode Floors
The game's rooms folder contains Greed Mode layouts for all of the alternate path floors (including Mortis). They can be accessed in-game by using the debug console and using the stage command to change the stage, by appending "c" or "d" to the stage number. These appear to have been scrapped in the middle of being designed—Downpour, Dross, Mines, Ashpit, and Mausoleum are populated with room layouts and enemy waves, but Gehenna, Corpse, and Mortis have no enemy waves; initiating the enemy sequence will only cause empty waves to appear that end immediately. Item rooms work as expected, providing one visible choice and one blind choice. Because these floors are unfinished, however, there are some oddities and strange occurrences:
- All boss waves on these floors are assigned the default difficulty level of 1. If the game attempts to spawn a boss wave with a higher difficulty, it will spawn an empty wave, which ends the round instantly.
- All room layouts for the enemy arena in the Mines and Ashpit contain rails, but no enemy waves use minecarts. They can only be used by Hornfel, who only appears in the Mines.
- Great Gideon appears in a boss wave in the Mines and Ashpit, but the game crashes when he attempts to summon enemies.
- Exorcists appear in a few enemy waves in the Mausoleum, despite not appearing as enemies normally.
- Some room layouts in the Mausoleum contain enemies with the ID 306.1.1. This ID is a subtype of the Lil Portal, though it does not appear to behave any differently.
