According to the quest-naming scheme in the document "W3 Quest Asset List", the following sidequests were originally part of the main questline:

According to the documents W3 Quest Asset List and Prima Games Official TW3 Guide, the following quests were deleted from the main questline:
q204 - thralls
thralls was to be a Skellige quest involving a ritual in the holy grove. It would have featured Ermion (called Mousesack), Holgar, Hjord, Yennefer, and a deleted character known as Hvitr, a mildly mentally disabled albino boy.

Altered Main Quests
q501 - On Thin Ice
In the final game, during the quest On Thin Ice, Geralt and his allies repel the Wild Hunt's naval assault on Undvik.
However, according to the document W3 Locations production list, q501 would have originally seen the Wild Hunt attack Novigrad.
q502 - Tedd Deireadh, The Final Age
In the final game, during the quest Tedd Deireadh, The Final Age, Geralt races to the tower of Tor Gvalch'ca on Undvik after hearing from Eredin that Avallac'h is a traitor bent upon using Ciri for his own ends. Upon arrival, he finds out from Avallac'h and Ciri that his foe was lying, and Ciri enters the tower alone to defeat the White Frost.
However, according to the document W3 Locations production list, q502 would have originally seen Geralt enter Tor Gvalch'ca and battle a boss.
q504 - Something Ends, Something Begins II
In the final game, during the quest Ending - Ciri Becomes Empress, Geralt meets up with Ciri in a wintry White Orchard, where the two battle a Forktail and go fishing.
However, according to the document W3 Locations production list, q504 would have originally featured the lair of a Korred, a removed non-contract monster.
