These are some NSMBHD PMs sent sent to me from Simpson55 in 2017, in chronological order, with some irrelevant PMs and text omitted (the latter with "[...]").

[...]it's a tad blurry but here's what I can at least remember.

What I actually saw was a video recorded by someone who had the chance to play the "field" and "fortress" levels (This guy [...] used to work for a former french video game magazine called joystick, joypad or something like that, I have no idea of what he does now). I actually joined the beta revival project because I thought I could be of good help but I failed at finding the aforementioned video, and trust me if I had more photos (good quality ones), I'd have shared them too. As for the title screen, I don't know if it counts but there was that screen with the logo on a white background when no one was playing. From what I know there were three levels but I can only assure you that there were at least two, like I said, the "field" one and the "fortress" one. There were no large screens, only a Nintendo DS. And last, well, I really can't tell you much about the pipe cannons.

I realize my answers aren't of great use, back then I was way more excited about the newcoming Mario Kart DS and Mario and Luigi 2 than NSMB, if only I knew I'd kill to have the chance to rewatch those pictures 10 years later
I don't have any picture or video of these two games but feel free to ask me anything.
Back to NSMB. He played the demo at the E3 in LA but he didn't have any physical copy of the game.
There wasn't any website, it was a regular paper made magazine. I don't remember anything about it, I know there was once an article about Mario Party 6 but that's it.

He took it at the E3 itself.
All I can say about Mario Kart DS's demo is that Yoshi circuit and Luigi Circuit from the GCN version were playable.
As for Mario & Luigi 2, the goal was to find all the hidden stars in a level. There were no time holes, instead, to go from Peach's Castle to a level, there were blue pipes.
