The NES Blockout is a port of the 3D arcade block-dropper, for which the NES didn't seem to have enough buttons (unless you count Start).

Two prototypes are known to exist, having turned up in a Goodwill of all places. This page currently covers the one designated as "7.05 Sample Version 1" on the title screen.
