The January 2019 build of Pizza Tower is the 1st Patreon build. This build introduces Desert (this level would stay used for a long time until it became Oregano Desert), Mansion (which had some changes before getting scrapped), & Factory (which had a old Demo 2 version beforehand, and eventually became Peppibot Factory), as well as a health meter that got popularized with the GOLF demo.

- "Demo 2" was removed from the title screen.
- All leftover areas from previous builds were removed from the data.
- This is the last build to feature the costumes from Demo 2, with the exception of the red clothes, which were replaced with Peppino's colors from the Grinch Race build.
- Peppino's slap move was changed to a grab attack, done shortly after the release of Demo 2.
- It was temporarily disabled in the Grinch Race build, but re-enabled here.
- When Peppino has grabbed an enemy, the player can press the Dash button to run forward with the enemy as a meat shield.
- The penultimate room of the PP_room tutorial was changed to not be so basic, now showing the player the ability to use the wallrun to climb up walls.
- Peppino's wallrun was reverted to how it worked in the Halloween Demo (in Demo 2, Peppino couldn't jump out of a wallrun).
- The Entrance tileset now uses softer colors.
- The hub room exclusive to this build plays "Theatrical Mischief".
- Desert uses a more blockier tileset, and is shorter than it would be in later builds.
- To activate Pizza Time, the player has to reach the end of the level and get the treasure.
- Mansion plays "Hot Spaghetti", rather than "Cold Spaghetti" as it would in the later March and April builds.
- The room with the key has a cheeseball that rolls down to the bottom of the room, which would be removed in later builds.
- Factory's layout has been reworked, hosting a far-more linear path than in Demo 2. "Peppino's Sauce Machine" is also the only track that plays in this level.
- There is no bonus reward for getting S ranks on all three levels.
- When pausing and selecting "Exit", the player will be booted back out to the hub room, rather than the title screen.
