Dubbing/Localization Oversights
Kasumi Dialogue Cutoff During School Cleaning Event

On 5/30, during the school cleaning event, Kasumi formally introduces herself to Joker. It seems as though her line is cut off after "my-" when a girl walks by and Kasumi jumps up to save her balloon, but if one is to open up the log and replay the line, it is revealed that there's more to the line than what is normally heard during normal gameplay. What is normally not heard yet fully voiced after "I believe you already learned from our meeting at school that my-" is "last name is Yoshizawa, but my full name is-". It seems as though Kasumi was originally meant to speak this full line and instead save the girl's balloon after "is-" gets cut off, but this may have been cut shorter in English to be in line with the way the likely more concise Japanese audio track is synced up to the event.
Error in Thieves Den Gallery
Sumire's leotard in the bad ending credits is pink, while the Thieves Den gallery, as a full picture insert, puts it as blue. This error (or correction, as Sumire's leotard is blue in all of its appearances) is only in the international releases; the Japanese release keeps the pink leotard.

Akechi's Critical Cutin Error
Akechi (Black Mask)'s cutin uses his prince version's alpha mask. The Thieves Den's gallery image and PC version of the 2022 remaster shows the correct render of it.

Safe Room Idle Animation
Akechi (Black Mask)'s idle animation has his right hand displaced. This bug affects his Prince Suit, Dark Suit, Winter Uniform, Winter Clothes, Gekkoukan High Uniform, Yasogami High Uniform, Kasugayama High Uniform, Butler Suit, Shadow Ops Uniform, and New Cinema Outfit. This was fixed in the 2022 remaster.

The portraits of the main characters are redrawn in Royal, but in a few scenes, the original portraits show up.

P3 and P4 Hero Textures
The texture files for the P3 and P4 Hero fought in DLC Velvet Room battles have textures for Ryuji's Persona Q2 costume.
Maruki Palace Flashbacks
When discussing with Lavenza about Maruki's Actualization, one flashback shot taking place in Maruki's Palace is different from what actually happened, seemingly from an earlier version of this area. Also, Maruki's earlier Palace Ruler design can be seen.

When viewing Ann/Morgana's Showtime from the Thieves Den, a mouse cursor is shown around the top-left corner.

A duplicate horse statue can be found under the room in one of Maruki's flashback scenes.
Hidden Test Track Room Door

There is a non-functional hidden door in the Test Track room of Maruki's Palace, which may have been from an earlier revision of the area.
Jazz Jin Room
There is a hidden small storage room in the Kichioji jazz club, where it is never seen in view. It seems to look a bit unfinished, and may have intended to be seen from the right side.
Futaba's Palace Old Room Leftover

The old room from Futaba's Palace, which was replaced by a Will Seed room, exists under it.
