Regional Differences
Several changes were made to the North American version, either to meet Nintendo of America's acceptable content guidelines or to make the game a little more visually appealing. These changes were retained in the later European releases.

Text
In the Japanese version, the text is spoken in the first-person perspective of Ace Harding. In the international versions, it's Ace's conscience... or something.

The international releases don't mention an electric chair, presumably at Nintendo's request.
Syringe/Pills
The syringe, as per Nintendo's policies of the time, was changed to medicine capsules in the international releases (which would explain why you never seem to run out).
The Ladies Room
The ladies room was labeled as such in the Japanese version.

The casino was almost completely redrawn for the international releases, giving it more vibrant casino-like colors, correcting the perspective on the slot machines, and fixing up the oddly-drawn roulette wheel.
Alligator
