MSTR means it's the "master" version of the game, and the numbers are the date, so this version was finished on October 12, 1989.

Due to a bug in the game's programing, the credits aren't displayed correctly. It is unknown if there was another version of the game that fixed that issue. However, hackers have managed to make the game display the graphics correctly. It's just a string that says "Programmer Kazuhiro Aoyama".
